A latex agglutination test for lectin binding.

Blood group A + H substance purified from hog gastric mucin was fixed to polystyrene particles by physical absorption to produce a lectin-agglutinable latex. Agglutination of this latex by soybean agglutinin, concanavalin A, and wheat germ agglutinin is specifically inhibited by the respective monosaccharide haptens and is not affected by known noninhibitory sugars. The sensitivity of the assay, a function of particle loading and latex concentration, approaches that of the hemagglutination test. NaCl, NaHCO3, NH4CO3, NH4-acetate, and Tris-HCl up to a concentration of 1 M do not interfere with the test. The latex suspension is stable for several months and can be stored in freeze-dried form.

We studied the utility of performing a penicillin binding protein 2a latex agglutination (PBP-LA) assay directly on Bactec blood culture broth samples containing Staphylococcus aureus to rapidly detect methicillin resistance. The sensitivity, specificity, positive predictive value, and negative predictive value of this method were 94.1%, 97.5%, 98%, and 92.9%, respectively.

The penicillin binding protein 2a (PBP2a) latex agglutination test using a blood culture pellet was compared to the oxacillin screen agar method using isolated colonies. For blood cultures positive for Staphylococcus aureus (n = 70), the direct PBP2a test was 18% sensitive and 100% specific. The PBP2a test shows poor sensitivity when used directly with positive blood cultures.
